Description An account of the resource In March 2004, the Irish Hospice Foundation began a two-year pilot project at Our Lady of Lourdes Hospital in Drogheda aimed at developing the hospice philosophy in a hospital setting. The project was informed from the findings of the ‘Care of the dying patient in hospital’ project conducted in St James’ Hospital and published in 1999. The purpose of the Care for People Dying in Hospitals Project was to pioneer a process for change in the culture of care and organisation with regard to dying, death and bereavement in hospitals. The project also aimed to enable the adoption of these changes in other hospitals. Research for the project pointed to tension between the needs of an acute hospital and the needs of dying people and their families. The limitations of the hospital’s physical environment, the shortage of private spaces, the need to develop skills for communicating in difficult situations, the work pressures on staff at all levels, and difficulties with recognising the dying state as a legitimate diagnosis were regarded as significant challenges. The project lead to the identification of the four themes which became the basis for the planning and development of the Hospice Friendly Hospitals programme; Integrated Care, Communication, Dignity and Design and Patient Autonomy. These themes would be the foundation for the Hospice Friendly Hospitals project. Mervyn Taylor was hired as the project manager for the ‘Care for people dying in hospitals’ project in December 2003 and would become the programme manager for the Hospice Friendly Hospitals programme which was developed from this pilot. The project won best hospital project at the Irish Pharmaceutical Awards 2005. It was also chosen as one of the three projects to represent Ireland at the 4th Quality Conference for Public Administrators in the EU in 2006. Led by Therese Brady, the aim was to provide specialist training, workshops and expert talks for people working in bereavement support and to work towards common standard in bereavement care provision. The group began offering training workshops and an annual conference was first held in 1993. In 1994 they began training for people to working with bereaved children and adolescents, in 1999 this course would be taken over by the Irish Hospice Foundation and developed into the Professional Certificate of Children and Loss. Description An account of the resource In 2006 following the success of the Care of people dying in hospitals pilot project in Our Lady of Lourdes Hospital, Drogheda the Irish Hospice Foundation was awarded funding from Atlantic Philanthropies to develop a national Hospice Friendly Hospitals programme. The HFH programme was undertaken in partnership with the HSE and a range of public service and academic organisations. The programme was initially developed as a five-year work-stream (2007-2012) designed to improve the quality of end-of life care in acute and community hospitals in Ireland. It was implemented across two ’phases’: 2007-10; 2010-12. The national programme was launched formally on 14 May 2007, by President Mary McAleese in St Mary’s Hospital in Dublin’s Phoenix Park. Central to the programme was the development of a comprehensive framework of standards regarding dying, death and bereavement in Ireland. Key Achievements; -The launch of the Quality Standards for End-of-Life Care in Hospitals by Minister of Health & Children, Mary Harney TD, in May 2010. -The HFH Programme won the 2010 Irish Medical Times Award for ‘Excellence in Healthcare Management’ -Commitment from HIQA to include the Quality Standards as a ‘core care process’ in their standards -A National Audit of End-of-Life Care in Hospitals, which included 1,000 patient deaths across 54 acute and community hospitals (comprising 75% of national acute bed capacity). This was the first audit of its kind in the EU. -The publication of Design & Dignity Guidelines for Physical Environments of Hospitals Supporting End-of-Life Care and subsequent development of the design & dignity programme to fund refurbishment projects in hospitals. -Engagement with the HSE Office of the CEO to secure a specific commitment in the HSE Service Plan for 2011 to the implementation of End-of-Life Care Development Plans in all hospitals. -The establishment of Networks of Champions with representatives from HSE local health offices, acute and community hospitals, primary care services and voluntary organisations to influence changes at policy level and to share information and good practice. -The development and implementation of ‘Final Journeys’ an interactive staff development programme aimed at all hospital staff. Description An account of the resource In March 2004, the Irish Hospice Foundation began a two-year pilot project at Our Lady of Lourdes Hospital in Drogheda aimed at developing the hospice philosophy in a hospital setting. The project was informed from the findings of the ‘Care of the dying patient in hospital’ project conducted in St James’ Hospital and published in 1999. The purpose of the Care for People Dying in Hospitals Project was to pioneer a process for change in the culture of care and organisation with regard to dying, death and bereavement in hospitals. The project also aimed to enable the adoption of these changes in other hospitals. Research for the project pointed to tension between the needs of an acute hospital and the needs of dying people and their families. The limitations of the hospital’s physical environment, the shortage of private spaces, the need to develop skills for communicating in difficult situations, the work pressures on staff at all levels, and difficulties with recognising the dying state as a legitimate diagnosis were regarded as significant challenges. The project lead to the identification of the four themes which became the basis for the planning and development of the Hospice Friendly Hospitals programme; Integrated Care, Communication, Dignity and Design and Patient Autonomy. These themes would be the foundation for the Hospice Friendly Hospitals project. Mervyn Taylor was hired as the project manager for the ‘Care for people dying in hospitals’ project in December 2003 and would become the programme manager for the Hospice Friendly Hospitals programme which was developed from this pilot. The project won best hospital project at the Irish Pharmaceutical Awards 2005. It was also chosen as one of the three projects to represent Ireland at the 4th Quality Conference for Public Administrators in the EU in 2006.
